MidNight Productions Membership Form

The form was created and design by Production Designer Manuel Franco C Tomeldan. It was now the official form for those who are Productions Current Project bands. Only the band leader will be the responsible for filling up the details. For those who have their bands and want to participate in every GIGS and Be part of the Project Bands you need to ask for Membership Form at Our Production Manager.

And 2x2 Picture and a check box to hereby accept the term & Condition of The Production. Last, The Printed Name of Band Leader and over it is a signature.

The Goal of the Production are to unite every Musicians, Rockers, Express their Talents, Meet New  Friends, To spread that music is part of everday life. As you dont know the prodution team goals for the future is to help charities, if we(us) succeed to pursue our goal we may do our best and tirelessly help other who are indeed in need.
